ACCESSING SERVICE

Begin attending Open Door 1 with just a few simple clicks.

Get Set Up

We operate an open referral system. Add your name, email address and preferred contact number and we’ll call back on Mondays or Wednesdays between 5 and 6 pm. This is excluding Bank Holidays and Christmas closures.

This will be a simple conversation with the manager directly or through your associated worker, such as a Social Worker, Psychiatric Worker, Nurses, Community Care Officers, Community Psychiatric Nurse (CPN) and Admiral Nurses. In this case, we will be in touch with the referee (GP, carer or health service or practitioner) and send forms to access your information.

We will complete a short form to get to know all about you. You can also meet with the Service Coordinator to confirm prices, dates of attendance, review your information, transport, dietary requirements and understand more about the service.

Changes to Your Contract

The contract may be terminated if the service provided by Open Door 1 CIC is no longer suitable/appropriate, following consultation with the user of the service, carer or any other interested parties. Deterioration of Service User, Persons entering full time care, Death/incapacitation of Spouse/Partner and measures taken to enable the Service User to stay in day service. These are dealt with through a person centered approach and will be tailored to the individual. Every step will be taken to maintain the independence and wellbeing of the individual involved.
